Prescription Information

Please bring with you to your appointment your pill bottles or a list of your current medications with doses, expiration dates, and number of refills available.

Refills

If you need to refill a prescription, please give 48 hours notice. In order to complete your request, we must have seen you within the past 6 months; 3 months if you are a patient with Diabetes. If you haven't been seen in that time frame, we will gladly set you up for an appointment to review your health and medications.

If you are taking any controlled substances, you will be rquired to schedule regular follow-up visits, usually on a monthly basis as per NYS law.
